Take Control of your Health! - Self Management Programs for Chronic Disease (CDSMP) and Diabetes (DSMP)
These complimentary programs are six-week, community-based workshops that empower adults and/or their caregivers to manage their chronic health conditions. Participants in this evidenced-based program are better able to manage their symptoms and have improved quality of life.
Chronic Disease Self-Management Program
This free program is for adults who wish to learn self-management techniques for any chronic condition. It teaches you how to deal with frustration, fatigue, isolation, and poor sleep. It gives exercises for maintaining and improving strength and flexibility, and covers communication with family, friends, and health professionals.

Diabetes Self-Management Program
Learn decision-making strategies, how to become a self-manager of your health, understanding common symptoms of diabetes, mindfulness tools to manage symptoms, exercise tips for strength, flexibility, and endurance, healthy eating tips at home and on the road, and how to manage your medications. Program is free.

Prevent T2 – Diabetes Prevention Program
Small Changes Can Prevent Diabetes
Prevent T2 has been shown to help at-risk adults cut their risk of developing type 2 diabetes by 58%. Did you know that one out of three adults have prediabetes and 90% of them don’t know it? The Prevent T2 program offers a proven approach to preventing or delaying the onset of type 2 diabetes through modest lifestyle changes made with the support of a coach and one’s peers.

Freedom from Smoking: Tobacco Cessation Group Program
This is a 7 week, 8 session, closed group (spots are limited and capped) highlighting the American Lung Association’s Freedom from Smoking program. This is an evidence based program that can help you quit all tobacco products, including e-cigarettes and vaping devices. This program will help you develop a personalized plan of action to prepare you for your quit day and offer the support you need to remain tobacco-free for life!
$10 materials fee.

Parkinson's Support Group
A community support group in where members can share their experiences, knowledge, and coping strategies. The benefits of social support in the management of chronic disease can empower and lift the morale of patients, friends, and community members, increase a sense of belonging, and enhance hope and sense of purpose through encouragement.

Menopause Support Group
Navigating menopause can feel overwhelming, but you don't have to do it alone. The Center for Health's Menopause FREE Support Group is a safe space where women can share experiences, ask questions, and support each other. Whether you're dealing with physical symptoms or the emotional changes of mid-life or want to better understand the journey, this group offers a compassionate community.
Open to women at all stages of menopause—whether you're in perimenopause or post-menopausal—you'll leave feeling empowered and connected. Meets 3rd Wednesday of each month from 5:15-6:15 pm on Zoom.

Flourish Through the Menopause Transition

The Wise Mind Series
Did you know that your brain starts to change up to 20 years prior to any outward signs of problems? There are some things we cannot change and there are lots of things that we can!
In this free program, we will be joined by Registered Dietitian, Lisa Fligor, to focus on nutrition and brain health as we age. We will discuss the main components that make up the MIND diet and ways to practically implement these recommendations into your daily routine. This will include a brief cooking demonstration, recipes and of course, delicious taste testing.
