Weigh to Go Weight Management Program

Take Control of your Health! - Self Management Programs for Chronic Disease (CDSMP) and Diabetes (DSMP)

These complimentary programs are six-week, community-based workshops that empower adults and/or their caregivers to manage their chronic health conditions. Participants in this evidenced-based program are better able to manage their symptoms and have improved quality of life.

Chronic Disease Self-Management Program

This free program is for adults who wish to learn self-management techniques for any chronic condition. It teaches you how to deal with frustration, fatigue, isolation, and poor sleep. It gives exercises for maintaining and improving strength and flexibility, and covers communication with family, friends, and health professionals.

Diabetes Self-Management Program

Learn decision-making strategies, how to become a self-manager of your health, understanding common symptoms of diabetes, mindfulness tools to manage symptoms, exercise tips for strength, flexibility, and endurance, healthy eating tips at home and on the road, and how to manage your medications.

Prevent T2 – Diabetes Prevention Program

Small Changes Can Prevent Diabetes

Prevent T2  has been shown to help at-risk adults cut their risk of developing type 2 diabetes by 58%. Did you know that one out of three adults have prediabetes and 90% of them don’t know it? The Prevent T2 program offers a proven approach to preventing or delaying the onset of type 2 diabetes through modest lifestyle changes made with the support of a coach and one’s peers.

Freedom from Smoking: Tobacco Cessation Group Program

This is a 7 week, 8 session, closed group (spots are limited and capped) highlighting the American Lung Association’s Freedom from Smoking program. This is an evidence based program that can help you quit all tobacco products, including e-cigarettes and vaping devices. This program will help you develop a personalized plan of action to prepare you for your quit day and offer the support you need to remain tobacco-free for life!

Caregiver Support Group

Share in a safe, supportive setting and learn ways to ease the stress of caregiving, as well as strategies and skills to solve problems. Learn how to maintain your own health while providing the best care for a loved one.

A community support group in where members can share their experiences, knowledge, and coping strategies. Our Health & Wellness Navigators guide local residents and area visitors through Tahoe Forest Health System. Health & Wellness Navigators make it simple and want you to be satisfied with your patient experience. The navigators can guide you to the appropriate physician, program, or service that fits your needs, including TFHS providers or hospital departments and community health care providers, wellness programs, and resources.
