High School Athlete Well Child Check
Who: For any high school athlete, NIAA forms are now due yearly. If you haven't had a Well Child Check in the last 12 months, schedule one now. If you are unsure, call and ask us to check our records.
What: A comprehensive Well Child Check and physical, completion of NIAA form, vaccines, and teenage specific screening
When: June 8, June 15, July 20, July 27
Where: Tahoe Forest Pediatrics, 10956 Donner Pass Rd, Ste 310, Truckee
How: To schedule, call (530) 582-6205 and request a "Well Child Check for a High School Athlete"
Arrive Prepared! Download and complete the NIAA form. Bring a parent Arrive at least 15 minutes before your scheduled appointment.
