Community Health Advocates/Promotores (CHA) are public health workers and trusted community members who are part of the TFHS Care Team. CHAs act as liaisons between health care providers, social services and the community. They help patients access services and improve quality, health equity and cultural competence of health care.
CHAs help patients and community members achieve their best health!
- Provide community outreach and culturally-appropriate education.
- Lead support groups and workshops.
- Assist in accessing and navigating medical, social and financial resources.
- Provide advocacy on behalf of patients and families.
- Help patients understand and follow their treatment plan.
Who is eligible?
CHA’s work with anyone who has at least two of these needs:
- Medical needs that require ongoing care
- Disability
- Mental health needs
- Limited health comprehension/health literacy (English and Spanish)
- Transportation barriers
- Limited social supports
- Financial hardships (assistance with food, utilities, medication, health insurance, unemployment, housing, etc.)
